Published fees and policies are intended to facilitate our work together and to clarify our mutual expectations.

Fee Schedule

Effective as of October 10, 2022, and until further notice

Individual counseling, psychotherapy for adults and young adults (from age 18):
 50-minute session (office or telehealth)
 Payment by cash, check, credit card (preferred)
 $150/session (telehealth) – $160/session (at the office)
 Saturday sessions (by special arrangement): Fee+40%
 Intake session only: +$25 administrative charge and new account set up (office and telehealth)

Couples counseling, premarital counseling, marriage therapy, discernment counseling
 50-minute session (office or telehealth)
 Payment by cash, check, credit card (preferred)
 $150/session (telehealth) – $160/session (at the office)
 Saturday sessions (by special arrangement): Fee+40%
 Intake session only: +$25 administrative charge and new account set up

Families (3+ attendees) conjoint counseling with adult and/or child family members
 50-minute session (office or telehealth – 100 minutes recommended)
 Payment by cash, check, credit card (preferred)
 $170/session (office or telehealth)
 Saturday sessions (by special arrangement): Fee+40%
 Intake session only: +$25 administrative charge and new account set up

EMDR Trauma Treatment for severe emotional or physical trauma, delayed physical/sexual abuse recovery, severe PTSD
 50-minute session (office or telehealth)
 Payment by cash, check, credit card (preferred)
 $160/session (office only, telehealth not available)
 Saturday sessions (by special arrangement): Fee+40%
 Intake session only: +$25 administrative charge and new account set up

Clinical Hypnotherapy for a variety of symptoms and conditions
 50-minute session (office or telehealth)
 Payment by cash, check, credit card (preferred)
 $160/session (office only, telehealth not available)
 Saturday sessions (by special arrangement): Fee+40%
 Intake session: +$25 administrative charge and new account set up

LEGAL SERVICES
I do NOT routinely testify on behalf of clients in divorce or other civil causes. On a case-by-case basis, I may be available to serve as an expert witness. My fee for this service is $1,450/day.

LETTERS, TAX DOCUMENTS, INSURANCE/LEGAL SUMMARIES & EVALUATIONS
Payment by cash, check, credit card (preferred)
$160/hour of preparation

Practice Policies

TELEPHONE CALLS
You’re welcome to call me if you have an urgent matter to discuss in between sessions. Your credit card will be charged a prorated charge for calls that are longer than 10 minutes.

INSURANCE
I do NOT take insurance of any kind. I’m an out-of-network provider. Upon request, a “superbill” can be provided after each session, which you can use to file for out-of-network insurance reimbursement.

PAYMENT
Payment is required at the time of service. Forms of payment accepted: Cash, all major credit cards, and debit cards. A credit or debit card on file is required of all clients and will be collected at the first session.

NEW ACCOUNTS
A one-time account administration fee of $25 is applied to all new clients, payable with the first session.

CANCELLATIONS AND NO SHOWS
A professional time commitment is set aside and held exclusively for you. If you need to cancel or reschedule an appointment, please notify my office at (678) 554-5632 at least 24 hours in advance. If the change is made with less than 24 hours’ advance notice or if you do not keep your scheduled appointment and cannot provide a valid reason, you will be responsible for the full fee.

OFFICE AVAILABILITY
Our voice mail at (678) 554-5632 is monitored during business hours. If possible, your call will be returned the same day, or the next business day at the latest.

EMAIL COMMUNICATIONS
The office email is appointments @ bellagiopsych . com. To fully protect your confidentiality, you are advised to limit your use of this email to appointment requests and appointment management only. My personal email is drz @ drmarkzuccolo . com. You may send confidential information to this email address.

MENTAL HEALTH EMERGENCIES
If you feel that you and/or your relative are in immediate danger or that you or your relative are a danger to self or others, please dial 911 or contact a local psychiatric hospital, or go to the nearest hospital emergency room. Our office does not provide emergency mental health services.
